ABSTRACT

Objective:To observe the changes of lactate clearance rate (LCR) and serum polyligandosan-1 (SDC-1) in patients with septic shock complicated with acute respiratory distress syndrome (ARDS) and to evaluate its prognostic value.Methods:Patients with septic shock and ARDS who were admitted to the Respiratory Intensive Care Unit (RICU) of Zhengzhou Central Hospital Affiliated to Zhengzhou University from February 2021 to April 2022 were selected as subjects. The patients were divided into the survival group and death group according to their 28-day survival status. General clinical data and related indicators of patients in the two groups were collected and compared. The related factors influencing the 28-day death of patients with septic shock and ARDS were screened, and receiver operating characteristic (ROC) curve was drawn to evaluate the individual and combined forecast value of LCR and SDC-1 for the prognosis of patients with septic shock and ARDS.Results:Compared with the survival group, sequential organ failure score (SOFA) and acute physiology and chronic health status score Ⅱ(APACHE Ⅱ) at admission to RICU, the levels of 24 h Lac, 6 h SDC-1, 24 h SDC-1 and 72 h SDC-1 in the death group increased significantly (all P< 0.05), and the levels of 6 h LCR, 24 h LCR, 6 h OI, 24 h OI and 72 h OI significantly decreased (all P<0.05). Spearman correlation analysis showed that SDC-1 at 6 h, 24 h and 72 h was significantly negatively correlated with OI at corresponding time points (all P<0.05), and LCR at 6 h and 24 h was significantly positively correlated with OI at corresponding time points (all P<0.05). Multivariate Logistic regression analysis showed that SOFA score, 24 h LCR, 24 h SDC-1 and 72 h SDC-1 were the risk factors of 28-d death in patients with septic shock and ARDS (all P<0.05). The areas under ROC curve of each related factor were SOFA score, 24 h LCR, 24 h SDC-1 and 72 h SDC-1, which could predict the prognosis (all P<0.05). 24 h LCR combined with 24 h SDC-1 had the maximum area under the curve (AUC=0.805, 95% CI: 0.691-0.920, with a sensitivity of 75.0% and a specificity of 74.4%). Conclusions:24 h LCR, 24 h SDC-1 and 72 h SDC-1 are the risk factors of the 28-day death of patients with septic shock and ARDS. 24 h LCR combined with 24 h SDC-1 can improve the test efficiency compared with the single indicator.

ABSTRACT

Objective:To explore the role and preliminary mechanism of heparin-binding protein (HBP) in the development of acute pancreatitis (AP) through clinical analysis and animal models.Methods:(1) Clinical research: Blood samples were collected from AP patients admitted to the Second Affiliated Hospital of Anhui Medical University from January 1 to December 31, 2021 within 30 min of admission, including 20 patients with severe acute pancreatitis (SAP) and 20 patients with non-severe acute pancreatitis (NSAP). Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of HBP, syndecan-1 and hyaluronic acid (HA). Modified CT severity index (MCTSI), another 20 healthy volunteers were selected as controls (HC). Spearman correlation analysis was used to analyze the correlation between HBP and syndecan-1, HA and MCTSI. Receiver operating characteristic (ROC) curve was used to evaluate HBP to predict AP severity. (2)Animal experiment: The rat model of acute pancreatitis was prepared by intraperitoneal injection of L-arginine. In the normal control group (NC, n=8), the low molecular weight heparin (LMWH) intervention group ( n=8), and the acute pancreatitis group ( AP, n=8), the rats were euthanized 12 h later, and peripheral venous blood was collected to detect the levels of HBP, syndecan-1 and HA. Lung tissue and pancreas tissue were collected to observe the pathological damage, and the polysaccharide coating damage of vascular endothelial cells was observed under a fluoroscopy electron microscope. Results:The level of HBP at admission was significantly higher in the AP group than in the HC group, and the increase in the SAP group was more obvious. Correlation analysis showed that HBP was positively correlated with syndecan-1, HA and MCTSI. Animal studies found that the levels of HBP, syndecan-1 and HA in the AP group were significantly higher than those in the NC group. The pancreatic pathological score showed that the AP group was significantly increased, and the fluoroscopy electron microscope showed that the vascular polysaccharide coating was complete in the NC group, and the structure of the AP group was severely damaged. After LMWH intervention, the structure shedding and damage were significantly reduced, and the difference was statistically significant.Conclusions:HBP can promote the progression of AP, which is related to the destruction of the polysaccharide coating structure of endothelial cells and the increase of vascular permeability caused by HBP.

ABSTRACT

Objective:To investigate the application of endothelial glycocalyx degradation products in assessing the severity of pulmonary edema in patients with acute respiratory distress syndrome (ARDS).Methods:A prospective study was conducted to select patients diagnosed with ARDS at Wuxi People's Hospital from July 1, 2018 to December 31, 2019. The extravascular lung water index (EVLWI) was recorded within 2 h after admission by continuous cardiac output with pulse indicator. The indexes of glycocalyx degradation products syndecan-1 (SDC-1), heparan sulfate (HS), hyaluronic acid (HA) and the concentrations of inflammatory factors [blood tumor necrosis factor α (TNF-α), interleukin (IL)-6 and IL-10] were measured by enzyme-linked immunosorbent assay. Pearson correlation method was adopted to analyze the correlation of glycocalyx degradation products with EVLWI and inflammatory factors in ARDS patients. The patients were divided into the mild pulmonary edema group and severe pulmonary edema group according to EVLWI at the cut-off value of 10 mL/kg, and the differences of glycocalyx degradation products and inflammatory factors between the two groups were compared. Receiver operating characteristic (ROC) curve of the subjects were plotted to analyze the value of glycocalyx degradation products in determining the severity of pulmonary edema.Results:A total of 85 ARDS patients were enrolled. Pearson correlation analysis showed that SDC-1, HS, and HA were all positively correlated with IL-6, TNF-α, EVLWI (all P<0.05), but did not correlate with IL-10 (all P>0.05). Comparison of indicators between the mild pulmonary edema group (39 cases) and the severe pulmonary edema group (46 cases) showed that: IL-6[(33.63±3.43) ng/L vs. (39.99±4.64) ng/L], TNF-α[(43.38±6.05) ng/L vs. (50.79±7.35) ng/L], SDC-1[(494.13±47.23) ng/L vs. (563.50±56.36) ng/L], HS[(114.02±18.39) ng/mL vs. (138.93±17.02) ng/mL], and HA[(441.44±62.52) ng/mL vs. (546.23±85.24) ng/mL] were statistically different between the two groups(all P<0.05). Whereas, IL-10 [(24.37±10.11) ng/L vs. (28.75±11.98) ng/L] was not statistically different between the two groups ( P>0.05). ROC curve analysis showed that the combined prediction of SDC-1, HA and HS indicators was superior to the single indicator. The area under the ROC curve combining the three indicators was 0.928 (95% CI: 0.872-1.000), with a sensitivity and specificity of 87.5% and 86.7%, respectively. Conclusions:There is a positive correlation between glycocalyx degradation products SDC-1, HS, HA and EVLWI in ARDS patients. The application of these three glycocalyx degradation products can be used as a reliable indicators for judging the severity of pulmonary edema in ARDS patients.

ABSTRACT

This report presents the clinical, microscopic and immunohistochemical aspects of a case of proliferative verrucous leukoplakia (PVL) mimicking oral lichen planus (OLP) in a 66-year-old woman. We also review the literature reporting cases of PVL mimicking OLP, where we found a higher prevalence in women who do not consume tobacco or alcohol. The initial manifestation of lichenoid areas was around the age of 59, with the diagnosis of PVL being established on average 6 years later, while malignant transformation occurred in 8 of the 22 cases at an average of 3.7 years after the final diagnosis of PVL. We emphasize the need for a close follow-up of any patient presenting white lesions of the oral mucosa. Lesions that are clinically and microscopically compatible with lichenoid reactions or OLP must be investigated and differentiated from PVL, which has a worse prognosis.


Este relato apresenta os aspectos clínicos, microscópicos e imuno-histoquímicos de um caso de leucoplasia verrucosa proliferativa (LPV) mimetizando líquen plano oral (LPO) em uma paciente do sexo feminino de 66 anos. Também revisamos a literatura relatando casos de LPV mimetizando LPO, onde encontramos maior prevalência em mulheres que não consomem tabaco ou álcool, com manifestação inicial de áreas liquenoides por volta dos 59 anos, sendo estabelecido o diagnóstico de LPV em média 6 anos depois, enquanto a transformação maligna ocorreu em 8 dos 22 casos em média 3,7 anos após o diagnóstico final de LPV. Ressaltamos a necessidade de acompanhamento rigoroso de qualquer paciente que apresente lesões brancas da mucosa oral, devendo ser investigadas lesões clinicamente e microscopicamente compatíveis com reações liquenóides ou LPO e diferenciadas da LPV, que tem pior prognóstico


Este reporte presenta los aspectos clínicos, microscópicos e inmunohistoquímicos de un caso de leucoplasia verrugosa proliferativa (LVP) simulando liquen plano oral (LPO) en una paciente de 66 años. También revisamos la literatura reportando casos de LVP simulando LPO, donde encontramos una mayor prevalencia en mujeres que no consumen tabaco ni alcohol, con una manifestación inicial de áreas liquenoides alrededor de los 59 años, estableciéndose el diagnóstico de LVP en promedio 6 años después, mientras que la transformación maligna ocurrió en 8 de los 22 casos en un promedio de 3,7 años después del diagnóstico final de LVP. Resaltamos la necesidad de un seguimiento estrecho de todo paciente que presente lesiones blanquecinas de la mucosa oral, que las lesiones clínica y microscópicamente compatibles con reacciones liquenoides o LPO deben ser investigadas y diferenciadas de la LVP, que tienen peor pronóstico.

ABSTRACT

Abstract Objective The aim of this study was to evaluate the serum Syndecan-1 (SDC-1) levels in patients with immunoglobulin-A vasculitis (IgAV) in children and its relation with gastrointestinal involvements. Methods Sixty-eight children with IgAV and 48 healthy children were enrolled in this cross-sectional study. Clinical and related laboratory data were collected from a computerized hospital database. Serum SDC-1 was collected on admission prior to treatment. Results Forty-eight patients fully met the IgAV diagnostic criteria at admission (IgAV group), 20 patients with rash only and diagnosed IgAV during hospitalization (Purpura group). In IgAV group, 30 patients with gastrointestinal involvements (IgAV-GI group) and 18 patients without gastrointestinal involvements (IgAV-NGI group). SDC-1 serum levels were significantly higher in the IgAV group (86.37 ng/mL (IQR 59.16-117.14 ng/mL)) than in the controls (20.37 ng/mL (IQR 15.52-26.45 ng/mL)) and the Purpura group (32.66 ng/mL (IQR 14.87-49.89 ng/mL)). Additionally, SDC-1 (OR = 1.08) was independently associated with IgAV with a cut-off value (sensitivity and specificity) of 66.55 ng/mL (68.8%, 95.0%), and the area under the curve was 0.908. The serum SDC-1 levels of the IgAV-GI group (106.92 ± 50.12 ng/mL) were significantly higher than those in the IgAV-NGI group (67.52 ± 17.59 ng/mL). Logistic regression analysis showed that SDC-1 (OR = 1.03) was independently associated with IgAV-GI with a cut-off value of 89.39 ng/mL. Conclusions SDC-1 serum levels may mirror vascular endothelium injury and mucosal damage in IgAV. Its applicability as a surrogate biomarker in IgAV remains to be determined.

ABSTRACT

Background: Dentigerous cyst diagnosis is simple but several case reports have documented neoplastic changes arising from them. Ameloblastoma is a common benign Odontogenic tumor with an aggressive manner and a high rate of recurrence. Ameloblastic Carcinoma is the malignant counterpart of Ameloblastoma but usually difficult to be distinguished from each other’s. Hence, need for Immunohistochemical markers may help achievement of accurate diagnosis. Objectives: Evaluation of Epithelial and Stromal Syn1 expressions and their roles in tumorigenesis and biological behavior of Dentigerous cyst, Ameloblastoma and Ameloblastic Carcinoma. Methods: Tissue samples comprising of 54 archived histopathologically confirmed cases of (10 Dentigerous cysts, 29 Ameloblastomas and 15 Ameloblastic Carcinoma). The sections were subjected to Immunohistochemical staining according to a standard protocol using antibody to Syn1. Results: Stromal Syn1 expression was higher in Desmolpastic Ameloblastoma than other Conventional Ameloblastoma subtypes. Unicystic Ameloblastoma showed higher Stromal Syn1 than Dentigerous cyst. Ameloblastic Carcinoma showed the highest immune-reactivity to Stromal Syn1 than Conventional Ameloblastoma. While, Epithelial Syn1 immune-reactivity was weak. Conclusions: Desmoplastic Ameloblastoma behaves in a more aggressive manner than other subtypes.Stromal Syn1 are highly expressed in aggressive and malignant odontogenic tumors and could be used together as prognostic predictor tool for odontogenic tumors

RESUMO Objetivo: Analisar a correlação entre a lesão do glicocálix medida pelo nível sérico de sindecano 1 e as disfunções de órgãos avaliadas com o escore PELOD-2, assim como avaliar sua associação com a mortalidade em sepse pediátrica. Métodos: Realizou-se um estudo prospectivo observacional em um hospital terciário público. Sessenta e oito pacientes pediátricos, com diagnóstico de sepse segundo os critérios da International Pediatric Sepsis Consensus Conference, foram consecutivamente recrutados. Nos dias 1 e 5, realizaram-se dosagens dos níveis séricos de sindecano 1 e avaliação dos componentes do escore PELOD-2. Os pacientes foram seguidos por até 28 dias após o diagnóstico de sepse. Resultados: Em geral, o nível de sindecano 1 estava aumentado em todos os participantes, com nível significantemente mais elevado nos pacientes em choque (p = 0,01). O nível de sindecano 1 no dia 1 teve correlação positiva com o escore PELOD-2 no dia 1 e coeficiente de correlação de 0,35 (p = 0,003). Nos primeiros 5 dias após o diagnóstico de sepse, as alterações nos níveis de sindecano 1 tiveram correlação positiva com modificações no escore PELOD-2, com coeficiente de correlação de 0,499 (p < 0,001). Com utilização de um ponto de corte dos níveis de sindecano 1 no dia 1 ≥ 430ng/mL, a disfunção de órgãos (escore PELOD-2 ≥ 8) pôde ser predita com área sob a curva de 74,3%, sensibilidade de 78,6% e especificidade de 68,5% (p = 0,001). Conclusão: O nível de sindecano 1 no dia 1 teve correlação com o escore PELOD-2 no dia 1, porém não se associou com a mortalidade aos 28 dias. A disfunção de órgãos (PELOD-2 ≥ 8) pôde ser predita pelo nível de sindecano 1 nas primeiras 24 horas de sepse, sugerindo seu significante envolvimento na fisiopatologia da disfunção de órgãos associada à sepse.


ABSTRACT Objective: To analyze the correlation between glycocalyx disruption measured via the serum syndecan-1 level and organ dysfunctions assessed by the PELOD-2 score and to evaluate its association with mortality in pediatric sepsis. Methods: We performed a prospective observational study in a tertiary public hospital. Sixty-eight pediatric patients diagnosed with sepsis according to International Pediatric Sepsis Consensus Conference criteria were consecutively recruited. We performed measurements of day 1 and day 5 serum syndecan-1 levels and PELOD-2 score components. Patients were followed up to 28 days following sepsis diagnosis. Results: Overall, the syndecan-1 level was increased in all subjects, with a significantly higher level among septic shock patients (p = 0.01). The day 1 syndecan-1 level was positively correlated with the day 1 PELOD-2 score with a correlation coefficient of 0.35 (p = 0.003). Changes in syndecan-1 were positively correlated with changes in the PELOD-2 score, with a correlation coefficient of 0.499 (p < 0.001) during the first five days. Using the cutoff point of day 1 syndecan-1 ≥ 430ng/mL, organ dysfunction (PELOD-2 score of ≥ 8) could be predicted with an AUC of 74.3%, sensitivity of 78.6%, and specificity of 68.5% (p = 0.001). Conclusion: The day 1 syndecan-1 level was correlated with the day 1 PELOD-2 score but not 28-day mortality. Organ dysfunction (PELOD-2 ≥ 8) could be predicted by the syndecan-1 level in the first 24 hours of sepsis, suggesting its significant pathophysiological involvement in sepsis-associated organ dysfunction.

Resumo Introdução: Tumores de glândulas salivares são um grupo diversificado de lesões, com várias origens e comportamentos extremamente diferentes, resultam em distintos desfechos para os pacientes. Portanto, a necessidade de descobrir novos marcadores com a capacidade de predizer o comportamento de neoplasias de glândulas salivares benignas e malignas é crucial. O syndecan-1 é uma proteína da superfície celular com papéis significativos em vários aspectos da função tumoral. Sua expressão nas neoplasias das glândulas salivares, especialmente seu componente estromal, ainda não foi investigada. Objetivos: Avaliar a imunopositividade do syndecan-1 nos componentes epiteliais e estromais das neoplasias de glândulas salivares e compará-la entre os subtipos benigno e maligno, além de avaliar sua correlação com os parâmetros clínico-patológicos. Método: Foram corados 133 tumores de glândulas salivares imuno-histoquimicamente com syndecan-1 e a intensidade e porcentagem dessa proteína foram determinadas, comparadas entre as lesões e correlacionadas com fatores clínico-patológicos. Resultados: A análise estatística das lesões com tamanho amostral suficiente mostrou diferenças significantes em porcentagem e intensidade entre os componentes epiteliais e estromais de todos os tumores (p < 0,05). As comparações pareadas demonstraram uma porcentagem de coloração significantemente maior das células epiteliais (p = 0,02) no tumor de Warthin em comparação com o adenoma pleomórfico e o carcinoma adenoide cístico. Da mesma forma, foram observadas intensidades de coloração e/ou percentagens significantemente maiores no carcinoma mucoepidermoide e no carcinoma adenoide cístico em comparação ao adenoma pleomórfico e ao tumor de Warthin (p < 0,05). Dos fatores clinico-patológicos, houve apenas uma correlação negativa significante entre o percentual estromal de carcinoma mucoepidermoide e a idade; e uma diferença significante entre a intensidade estromal + porcentagem de carcinoma adenoide cístico e sexo (p < 0,05). Conclusões: De acordo com nossos achados, o syndecan-1 estromal se correlaciona com o comportamento maligno de tumores de glândulas salivares, demonstra uma expressão mais alta, indica um papel para o syndecan-1 na invasão e metástase tumoral.

ABSTRACT

Objective:To investigate the value of plasma syndecan-1 (SDC-1) combined with lung ultrasonography in evaluating the degree of extravascular lung water in patients with acute respiratory distress syndrome (ARDS).Methods:From July 2018 to July 2019, 50 patients with ARDS admitted to the department of intensive care unit of Wuxi People's Hospital Affiliated to Nanjing Medical University were enrolled. After admission, pulse indicator continuous cardiac output (PiCCO) catheter was established for all patients. PiCCO indexes, including extravascular lung water index (EVLWI) and pulmonary vascular permeability index (PVPI) were monitored by one doctor. Another doctor performed lung ultrasound examination, and calculated the sum of the number of B-lines under 10 ultrasound sections of upper blue point, lower blue point, diaphragm point, Plaps point and rear blue point of both lungs. Then the level of plasma SDC-1 was detected by enzyme linked immunosorbent assay (ELISA). Pearson correlation method was used to analyze the correlation between the number of ultrasonic B-lines, plasma SDC-1 level and EVLWI and PVPI. Taking 10 mL/kg EVLWI as the boundary value, the degree of pulmonary edema in patients with ARDS was divided into mild pulmonary edema and severe pulmonary edema. The receiver operator characteristic curve (ROC curve) was drawn, and the number of B-lines, SDC-1 and the predictive value of the combination of the above two indicators on the severity of pulmonary edema in patients with ARDS were analyzed.Results:The cardiac index (CI) and central venous pressure (CVP) of 50 patients with ARDS were (46.84±6.00) mL·s -1·m -2 and (8.12±1.80) mmHg (1 mmHg = 0.133 kPa), cardiogenic pulmonary edema was excluded. In 50 patients with ARDS, EVLWI was (10.82±2.92) mL/kg, PVPI was 3.02±0.69, the number of ultrasound B-lines was 40.90±13.05, and plasma SDC-1 was (568.25±118.14) μg/L. Pearson correlation analysis showed that the number of ultrasound B-lines in patients with ARDS was significantly positively correlated with EVLWI and PVPI ( r1 = 0.802, r2 = 0.799, both P < 0.01). Plasma SDC-1 was also positively correlated with EVLWI and PVPI ( r1 = 0.732, r2 = 0.576, both P < 0.01). ROC curve analysis showed that the number of B-lines and SDC-1 had good predictive value for the severity of pulmonary edema in patients with ARDS. The area under ROC curve (AUC) and 95% confidence interval (95% CI) were 0.891 (0.803-0.979) and 0.875 (0.772-0.978), respectively. When the cut-off of B-lines was 40.50, the sensitivity and specificity were 82.1% and 86.4%, respectively. When the cut-off of SDC-1 was 559.37 μg/L, the sensitivity and specificity were 85.7% and 81.8%, respectively. Combining the number of B-lines with SDC-1 could further improve the predictive value of pulmonary water in patients with ARDS. The AUC (95% CI) was 0.958 (0.890-1.000), and the sensitivity and specificity were 92.9% and 91.8%, respectively. Conclusions:The level of plasma SDC-1 and the number of pulmonary ultrasonic B-lines have a good correlation with the degree of extravascular lung water in patients with ARDS. The combined application of the two noninvasive indexes can be used to evaluate the degree of extravascular lung water in patients with ARDS.

RESUMO Objetivo: Investigar se a hiperemia reativa correlaciona-se com marcadores de disfunção endotelial e pode ser utilizada para identificar sepse na doença crítica. Métodos: Trata-se de estudo prospectivo em uma coorte de pacientes críticos. A disfunção endotelial foi avaliada quando da admissão, por meio da quantificação de hiperemia por tonometria arterial periférica e níveis plasmáticos de endotelina 1, E-selectina solúvel, endocana e sindecano 1. Os pacientes sépticos foram comparados com pacientes sem evidência de infecção. Resultados: Cinquenta e oito pacientes sépticos foram comparados com 28 controle. O logaritmo natural da tonometria arterial periférica teve correlação negativa com comorbidades cardiovasculares, severidade da doença e níveis plasmáticos de E-selectina solúvel (p = 0,024) e sindecano 1 (p < 0,001). O logaritmo natural da tonometria arterial periférica foi mais baixo nos pacientes sépticos quando comparado com os de pacientes controle (0,53 ± 0,48 versus 0,69 ± 0,42, respectivamente) e, quando ajustado à idade, o modelo multivariado predisse que cada 0,1 de diminuição em unidades de logaritmo natural da tonometria arterial periférica levou a aumento de 14,6% na probabilidade de infecção. Conclusão: A hiperemia reativa avaliada por tonometria arterial periférica tem estreita relação com E-selectina solúvel e sindecano 1, o que sugere associação entre ativação endotelial, degradação de glicocálix e reatividade vascular. A hiperemia reativa por tonometria arterial periférica parece estar comprometida em pacientes críticos, especialmente os com sepse.


Abstract Objective: To investigate whether reactive hyperemia measured by peripheral arterial tonometry correlates with markers of endothelial dysfunction and may be used to identify sepsis in critical illness. Methods: A prospective study was performed using a cohort of critically ill patients. Endothelial dysfunction was assessed on admission by quantifying reactive hyperemia-peripheral arterial tonometry and plasma levels of endothelin-1, soluble E-selectin, endocan and syndecan-1. Septic patients were compared to patients without evidence of infection. Results: Fifty-eight septic patients were compared to 28 controls. The natural logarithm of reactive hyperemia-peripheral arterial tonometry was negatively correlated with cardiovascular comorbidities, disease severity and plasma levels of soluble E-selectin (p = 0.024) and syndecan-1 (p < 0.001). The natural logarithm of reactive hyperemia-peripheral arterial tonometry was lower in septic patients than in controls (0.53 ± 0.48 versus 0.69 ± 0.42, respectively). When adjusted for age, the multivariable model predicted that each 0.1-unit decrease in natural logarithm of reactive hyperemia-peripheral arterial tonometry increased the odds for infection by 14.6%. m. Conclusion: Reactive hyperemia-peripheral arterial tonometry is closely related to soluble E-selectin and syndecan-1, suggesting an association between endothelial activation, glycocalyx degradation and vascular reactivity. Reactive hyperemia-peripheral arterial tonometry appears to be compromised in critically ill patients, especially those with sepsis.

ABSTRACT

AIM: To analyze the effect of curcumin on the expression of transcription factors Twist1, Twist2, E-cadherin and Syndecan-1 in rats with endometriosis. METHODS: Thirty SD rats were randomly divided into five groups: model group, low dose curcumin group (25 mg•kg-1•d-1), medium dose curcumin group (50 mg•kg-1•d-1), high dose curcumin group (100 mg•kg-1•d-1) and gestrinone group (0.5 mg•kg-1•d-1). There were 6 rats in each group, and 5 rats in sham operation group were set up as blank control group. The expression of Twist1, Twist2, E-cadherin and Syndecan-1 was detected by immunohistochemistry. RESULTS: Compared with the model group, the ectopic tissue volume of curcumin group and gestrinone group was significantly lower, the difference was statistically significant (P0.05). CONCLUSION: Curcumin can reduce the expression of Twist1, Twist2 and Syndecan-1, increase the expression of E-cadherin, and inhibit the growth of endometrium in the model rats, so as to achieve the purpose of treatment.

ABSTRACT

Aim: Syndecan-1 (CD138) has most extensively been studied and correlated with many types of cancers. CD138 is mainly expressed in epithelial cells and plasma cells. Quite a few previous studies have called attention to the expression status of CD138 in EC. We aimed to determine the location of CD138 in the hyperplasia and endometrial carcinoma progression scale, and its relation with proliferation via Ki67, and to identify a new criterion in the differential diagnosis of hyperplasia–carcinoma. Materials and Methods: A total of 120 endometrial curettage materials with proliferative and secretory phase endometrium, hyperplasia without atypical, atypical hyperplasia, and endometrium carcinoma examined in the pathology laboratory between 1995 and 2016 were included in the study. Samples were subjected to immunostaining for CD138 and Ki67 using antibody. Results: Statistical analysis revealed a significant and negative correlation between histopathological progression and CD138 (P < 0.01). Statistical analysis revealed a significant and positive correlation between histopathological progression and Ki67 (P < 0.01) and a significant negative correlation between Ki67 and CD138 (P < 0.01). Conclusion: CD138 may be helpful in diagnostic difficulties, and benign pathologies should be considered due to the increase in staining density. Furthermore, we believe that CD138 will be an important criterion in determining the risk of malignancy in hyperplasia, and the risk of malignancy progression may increase in cases, of which intensity of staining decreases in accordance with the normal endometrium.

Background: Matrix metalloproteinase-7 (MMP-7), a member of the MMPs family, is expressed in injured mucosal epithelial cells of many organs. Syndecan-1 (SDC-1), a substrate of MMP-7, plays an important role in tissue repair and maintaining intestinal barrier function. Aims: To investigate the expressions and significance of MMP-7 and SDC-1 in inflammatory bowel disease (IBD). Methods: Forty-five intestinal mucosa samples and fifty peripheral blood samples from inpatients with active IBD were collected during Jan. 2017 to Jan.2018 at the Second Affiliated Hospital of Zhengzhou University. Thirty intestinal mucosa samples and thirty-five peripheral blood samples from non-IBD subjects were served as controls. Expressions of MMP-7 and SDC-1 in intestinal mucosal tissue were detected by immunohistochemistry, and serum SDC-1 level was determined by ELISA. Results: MMP-7 was highly expressed in intestinal mucosal tissue of IBD patients, especially in mucosal epithelium bordering the ulcer area. Compared with control group, the expression of MMP-7 was significantly increased and that of SDC-1 was significantly reduced in intestinal mucosal tissue of IBD patients (P<0.05). In contrast, the serum level of SDC-1 was significantly increased in IBD patients (P<0.05). Spearman correlation coefficient analysis revealed a positive correlation between MMP-7 expression and the disease activity of IBD (rs=0.519, P<0.05 for ulcerative colitis; rs =0.583, P<0.05 for Crohn's disease), and a negative correlation between MMP-7 and SDC-1 in inflamed mucosa (rs=-0.646, P<0.05). Conclusions: Increased MMP-7 expression in intestinal mucosal tissue may contribute to the initiation and development of IBD via cleaving SDC-1 and impairing mucosal healing.

ABSTRACT

Objective To evaluate the treatment effects of epidural labor analgesia on syndecan-1 in patients with preeclampsia.Methods During Jan.,2016 and Dec.,2017,150 unipara females with preeclampsia and gestational age>37 weeks were enrolled in the prospective cohort study.They were devided into intervention and control group (n =75 in each group) with and without epidural labor analgesia,based on whether epidural labor analgesia applications were made by the patients at the first examination of cervical dilation of 2 cm.The primary outcomes were mean change and change rate of syndecan-1 from baseline to 2 h post-treatment,and the secondary outcomes were visual analogue scale (VAS),IL-6 level and mean arterial pressure (MAP).Results Absolute change of syndecan-1 from baseline to 2 h post-baseline was (-11.49 ± 82.80) ng/mL in the intervention group and (45.09 ± 55.14) ng/mL in the control group,respectively.There was a significant difference in mean change of syndecan-1 between the two groups (P<0.001).Similarly,the intervention group exhibited a greater reduction in change rate of syndecan-1 compared with the control group (P<0.001).Moreover,the intervention group had greater reduction in VAS and MAP compared with the control group (P<0.001 for both).There was no significant difference in IL-6 level between the two groups.Conclusions Epidural labor analgesia was associated with greater reduction in syndecan-1,VAS and MAP compared with the control condition.In contrast,administration of epidural analgesia did not significantly change IL-6 level.

BACKGROUND: Syndecan-1 (sCD138) has recently been suggested to predict the clinical course of early-stage chronic lymphocytic leukemia (CLL), but few studies have been reported. This study assessed the role of syndecan-1 in the prognosis of patients with CLL and its correlation with other prognostic markers. METHODS: This prospective study was performed in the hematology department of an Indian tertiary care center, over nineteen months (Jun. 2009–Jan. 2011). Forty-nine new patients with CLL presented during this period and were included. Twenty age- and gender-matched healthy patients served as controls, and six patients with multiple myeloma were included as positive controls. Baseline serum syndecan-1 concentrations were measured for all patients at presentation using ELISA (Diaclone, Besancon, France). At baseline, patients were divided into low (N=10), intermediate (N=18) and high (N=21) risk cohorts. Serum syndecan-1 levels in these patient subgroups were compared with clinical and laboratory parameters. RESULTS: The median syndecan-1 level in patients with CLL (73.32 ng/mL, range, 28.71–268.0 ng/mL) was marginally higher than that in healthy patients (63.10 ng/mL, range, 55.0–75.11 ng/mL). At presentation, syndecan-1 levels in patients with CLL correlated strongly with symptomatic disease (cytopenias, P=0.004) and higher clinical stage (Rai stage III and IV, P=0.001) markers and poorly with β2-microglobulin level (P=0.270), diffuse BM infiltration (P=0.882), and surrogate mutation status markers (CD 38, P=0.174 and ZAP-70, P=0.459). Syndecan-1 levels dichotomized by the median value were higher with progressive disease markers, e.g. shorter lymphocyte doubling time (LDT, P=0.015) and increased treatment (P=0.099). CONCLUSION: In CLL, serum syndecan-1 (sCD138) levels at presentation correlate with disease burden, and higher baseline levels may predict early treatment.

ABSTRACT

Objective@#To investigate the expression of syndecan-1 and syndecan-2 and their clinicopathological significance in patients with gallbladder squamous cell (SC)/adenosquamous carcinoma (ASC) and adenocarcinoma (AC).@*Methods@#A total of 126 patients with SC/ASC (n=46) and AC (n=80) were included in this study. The expression levels of syndecan-1 and syndecan-2 were detected by Envison™ immunohistochemistry assay. The clinical and prognostic significance of syndecan-1 and syndecan-2 were analyzed.@*Results@#In the 46 SC/ASC samples, syndecan-1 and syndecan-2 were positively expressed in 29 (63.0%) and 28 (60.9%) tumor tissues, respectively. (Positive expression was defined based on the staining in the component of squamous cell carcinoma. That is to say, the tissue which adenocarcinoma part was positively stained, but squamous cell carcinoma part was negatively stained is also regarded as negative.) In the 80 AC samples, 47 (58.8%) cases showed syndecan-1 positive expression, and 51 (63.8%) showed syndecan-2 positive expression. There was no significant difference in the positive rates of syndecan-1 and syndecan-2 between SC/ASC and AC groups (P>0.05 for all). The levels of syndecan-1 and syndecan-2 were associated with tumor size, TNM staging, lymph node metastasis, invasion of adjacent tissue, and surgical procedures in SC/ASC patients (P<0.05 for all). However, their expression was associated with tumor differentiation, tumor size, TNM staging, lymph node metastasis, invasion of adjacent tissue, and surgical procedures in AC patients (P<0.05 for all). The Kaplan-Meier survival analysis of SC/ASC and AC patients revealed that the average survival time for patients with positive syndecan-1 and syndecan-2 expression was significantly shorter than that of those with negative expression (P<0.01 for all). Cox multivariate analysis indicated that syndecan-1 and syndecan-2 expression were independent unfavorable prognostic factors for SC/ASC and AC patients (P<0.05 for all).@*Conclusion@#The syndecan-1 and syndecan-2 expression are associated with the tumor progression and poor prognosis in patients with gallbladder SC/ASC and AC.

Objective To explore the changes and clinical significance of serum syndecan-1 and MMP-9 levels in patients with different stages of endometriosis (EMS).Method 89 patients diagnosed as EMS and meeting criterions were collected from Jul.2014 to Aug.2016 in our hospital,including 20 cases of stage Ⅰ,27 cases of stage Ⅱ,28 cases of stage Ⅲ,and 19 cases of stage Ⅳ.At the same time,42 healthy women in our hospital were selected as control group.General information of the two groups was comparable.The serum syndecan-1 and MMP-9 levels of EMS group and the control group were detected by ELISA in fasting state.Results The serum syndecan-1 and MMP-9 levels of EMS group were higher than those of the control group(P<0.05).The serum syndecan-1 and MMP-9 levels increased significantly with the increase of EMS stage (P<0.05).The serum syndecan-1 and MMP-9 levels of EMS group had no significant difference at proliferative phase and secretory phase (P>0.05).The levels of serum syndecan-1 (r=0.832,P=0.000) and MMP-9 (r=0.764,P=0.000) were positively correlated with EMS stage.There was a positive correlation between serum syndecan-1 and MMP-9 (r=0.684,P=0.000).Conclusion Serum soluble syndecan-1 and MMP-9 show high levels in patients with EMS,and the serum levels are not related to the menstrual cycle.They may be involved in the pathogenesis and progression of EMS through synergistic effects.

Objective To detect the level of serum syndecan-1 in patients with oral tongue squamous cell carcinoma (OTSCC),and to investigate the effect of serum syndecan-1 in OTSCC diagnosis and disease progression assessment.Methods The serums of a total of 65 OTSCC patients (OTSCC group) and 74 healthy subjects (control group) were collected from March 2015 to June 2017 in the Fourth Hospital of Xi'an.Enzyme linked immunosorbent assay was used to detect the level of serum syndecan-1.Receiver operating characteristic (ROC) curve was conducted to evaluate the diagnostic value of serum syndecan-1 in OTSCC and its disease progression.Results The level of serum syndecan-1 in OTSCC group was significantly lower than that in the control group [(91.87 ± 33.18) ng/ml vs.(162.32 ± 51.16) ng/ml,t =1.977,P =0.002].The serum syndecan-1 levels of different TNM stage (F =3.536,P =0.025),tumor invasive depth (F =1.254,P =0.039) and lymph node metastasis (t =2.420,P =0.018) in OTSCC group had statistically significances.ROC curve analysis showed that 95.54 ng/ml serum syndecan-1 [area under the curve (AUC) =0.944,P < 0.001,95% CI:0.915-0.974] was the cut-off value of OTSCC diagnosis,and 74.54 ng/ml of lymph node metastasis in OTSCC diagnosis (AUC =0.783,P < 0.001,95% CI:0.697-0.867).Conclusion Serum syndecan-1 level is markedly decreased in patients with OTSCC,and is valuable to diagnose OTSCC or its lymph node metastasis,which is worthy of clinical application.

@#AIM: To investigate the effects of insulin on syndecan-1 expression, cellular permeability and proliferation in human retinal microvascular endothelial cells. <p>METHODS: Cells were treated with 100nmol/L and 1 000nmol/L insulin for 48h respectively. Expression of protein and mRNA were detected by western blot and quantitative real-time polymerase chain reaction. Cellular proliferation and permeability were examined by methods of methylthiazolyl tetrazolium and horseradish peroxidase. <p>RESULTS: With treatment of insulin, protein and mRNA of syndecan-1 both increased obviously, and the effect of high level insulin was more significant. After treated with insulin, cellular proliferation and permeability both enhanced, and the effects of high level insulin were stronger. <p>CONCLUSION: Insulin can up-regulate syndecan-1 protein and mRNA in cultured human retinal microvascular endothelial cells, and increase cellular permeability and proliferation.

Nasal polyp (NP) is a common chronic inflammatory disease of the nasal cavity and sinuses.Although some authors have suggested that NP is related to inflammatory factors such as interleukin (IL)-1β,IL-5,IL-8,granulocyte-macrophage colony-stimulating factor (GM-CSF),tumor necrosis factor (TNF)-α,and IL-17,the mechanisms underlying the pathogenesis and progression of NP remain obscure.This study investigated the expression and distribution of IL-17 and syndecan-1 in NP,and explored the roles of these two molecules in the pathogenesis of eosinophilic chronic rhinosinusitis with nasal polyps (Eos CRSwNP) and non-Eos CRSwNP.Real-time PCR and immunohistochemistry were used to detect the expression of IL-17 and syndecan-1 in samples [NP,unciform process (UP) from patients with CRS,and middle turbinate (MT) from healthy controls undergoing pituitary tumor surgery].The results showed that the expression levels of IL-17 and syndecan-1 were upregulated in both NP and UP tissues,but both factors were higher in NP tissues than in UP tissues.There was no significant difference in IL-17 levels between the Eos CRSwNP and non-Eos CRSwNP samples,and syndecan-1 levels were increased in the non-Eos CRSwNP tissues as compared with those in Eos CRSwNP tissues.In all of the groups,there was a close correlation between the expression of IL-17 and syndecan-1 in nasal mucosa epithelial cells,glandular epithelial cells,and inflammatory cells,suggesting that IL-17 and syndecan-1 may play a role,and interact with each other,in the pathogenesis ofnon-Eos CRSwNP.
